Why do you need a medical questionnaire completed?

It is important that our instructors have your medical information so that they can ensure their classes are safe for you and where applicable adjustments are made.   We ask you to keep your instructor up to date of any changes so they can continue to meet your needs.  It may be necessary for you to ensure your medical practitioner is supportive of your attendance before starting a class.
